Optimization Methods Of Using Local Herbal Preparations In The Treatment Of Severe Chronic Generalized Periodontitis.

. In recent years, the use of local medicinal plants for the treatment of oral cavity diseases (OC), including periodontal tissues (PT), has gained particular importance. In the republic, the development of phytopreparations that are environmentally friendly, high-quality, and economically accessible holds significant value. Additionally, the justification for their widespread use in medicine, including in the treatment of dental diseases, is crucial. Demonstrating their advantages, as well as developing unified scientific and practical approaches and treatment algorithms, is of great importance today.
The aim of the study: To scientifically and practically substantiate alternative approaches to the treatment of severe chronic generalized periodontitis (SCGP) using the phytopreparations "Hemostat" liquid extract and "Clove Essential Oil" tincture.
Materials and Methods. The "Hemostat" liquid extract, "Clove Essential Oil," and a 0.05% solution of the synthetic preparation "Chlorhexidine Bigluconate" were used individually and in combination for 326 patients diagnosed with severe chronic generalized periodontitis (SCGP, K05.5). The condition of periodontal tissues (PT) was assessed based on 8 clinical criteria using a special chronological chart before treatment, as well as 10 and 20 days after the start of treatment. The study included oral hygiene indices (CPITN, PMA, PI), quantitative and qualitative characteristics of the microflora in periodontal pockets, neutrophil infiltration, local allergic reactions, bleeding in periodontal pockets and sulci, tissue repair, and atrophy of interproximal alveolar bone structures.
Results. An evaluation of the clinical condition of periodontal tissues (PT) based on criteria before treatment, and at 10 and 20 days after treatment, revealed that the synthetic preparation "Chlorhexidine" was the most "problematic" and "less effective" compared to the phytopreparations "Hemostat" and "Clove Essential Oil." However, it was noted that in terms of positive outcomes for PMA and PI indices, "Chlorhexidine Bigluconate" was not inferior to the phytopreparations. Furthermore, it was emphasized that the phytopreparations contribute to improving PT viability, reducing local allergic damage, controlling microbial species in periodontal pockets (PP) and sulci, and decreasing manifestations of local allergic reactions.
Conclusions. Observations based on 8 indicators during the treatment of severe chronic generalized periodontitis (SCGP) allow us to conclude that the clinical effectiveness of the integral indicators for eliminating the pathology supports the possibility of applying alternative methods when selecting oral antiseptics. The phytopreparation "Hemostat" liquid extract proved to be the most effective, followed by "Clove Essential Oil," and with satisfactory results, the synthetic preparation "Chlorhexidine Bigluconate." This led to the final conclusion regarding their effectiveness in treatment.
