The most common reason for endodontic treatment failure is the clinician's inability to find the extra root and canal present; however, sometimes there may be fusion of roots resulting in less number of roots and canals present in a tooth, especially in molar teeth. As a result, the endodontist must carefully inspect the root canal system, preferably under magnification of Dental Operating Microscope. The use of modern imaging techniques like cone beam computed tomography to diagnose such abnormalities is strongly recommended. The root canal treatment of a rare maxillary first molar under Dental Operating Microscope with a single root and a single canal is highlighted in this case report.
