Urolithiasis, also known as renal stone disease, is a global condition affecting the urinary system and is a common painful disorder that causes urinary retention and renal colic. In Ayurveda, it is correlated with Ashmari, for which Ashmarihara kashaya is described as a therapeutic option. Unlike modern surgical or conservative treatments that are often associated with complications, Ayurveda offers a holistic, non-invasive approach. A literature search was conducted using Ayurvedic research databases and academic journals including AYU, IRJAY, WJPPS, JCDR and WJPR, and evidence from clinical trials, case studies, and drug reviews highlights the clinical applications and potential efficacy of Ashmarihara kashaya in managing early to moderate cases of urolithiasis. Studies such as the management of Mutrashmari with Palasha Kshara and Ashmarihara Kwatha, clinical evaluation of Ashmarihara Kashaya, case reports on multiple renal calculi with hydronephrosis, and drug reviews suggest its role in reducing stone burden and alleviating symptoms. However, most of these studies were conducted on small sample sizes, and larger-scale research is needed to validate and generalize the findings.
