Despite the extensive historical use of Ayurvedic medicine, scientific validation of its pharmacological mechanisms remains limited. Srikhandasava, a fermented polyherbal formulation centered around Santalum album, is traditionally used for the management of alcohol addiction and related toxicities. Comprising multiple herbs with known therapeutic properties, the formulation undergoes controlled fermentation, enhancing the bioavailability and potency of its constituents. Srikhandasava is indicated for improving immunity, digestion, respiratory health, and mitigating oxidative stress, suggesting potential anti-inflammatory and antioxidant effects. This review highlights the necessity for systematic scientific studies to elucidate the mechanistic pathways of Srikhandasava, aiming to integrate traditional Ayurvedic wisdom with contemporary biomedical frameworks.
