A 30-year-old female with a third-degree atrioventricular block (AV block), insulin-dependent diabetes mellitus (Type 1), chronic progressive external ophthalmoplegia (CPEO), and a history of three failed pregnancy attempts presented with breathlessness one day ago. The patient was diagnosed with a third-degree atrioventricular block, necessitating an emergency lower segment cesarean section. The patient was treated under general anesthesia to keep the hemodynamics stable along with temporary pacing, resulting in the successful delivery of dichorionic diamniotic twins. This case underscores the importance of a multidisciplinary team approach and timely interventions in reducing maternal and neonatal mortality in such complex disorders.
