Abstract
Maxillary first molars typically exhibit three roots and four canals. However, anatomical variations are common, including the presence of extra roots and canals. This case report presents a rare instance of a maxillary first molar with four distinct roots and canals. The patient presented with severe tooth pain and was successfully treated with endodontic therapy. This case highlights the importance of thorough preoperative radiographic evaluation to identify such anatomical variations and its successful management. By recognizing and effectively managing these morphological anomalies, clinicians can improve treatment outcomes and patient satisfaction.
