Abstract

Background: The relationship between orthodontic treatment and gingival recession is complex and multifaceted, with various studies yielding differing results. Understanding the factors contributing to gingival recession during orthodontic procedures is crucial for optimizing patient outcomes and minimizing adverse effects. This study was conducted to systematically review and synthesize the evidence regarding the impact of orthodontic treatment on gingival recession and identify key factors influencing this relationship.

Methods : A systematic search of multiple databases was conducted to identify studies examining the relationship between orthodontic treatment and gingival recession. Inclusion criteria encompassed randomized controlled trials (RCTs), prospective clinical trials, and retrospective longitudinal cohort studies. Data were extracted on study characteristics, sample sizes, participant ages, orthodontic treatment types, and outcomes related to gingival recession.

Results: The systematic review included seven studies examining the relationship between orthodontic treatment and gingival recession. The findings indicated that excessive tooth proclination, particularly of the lower incisors, significantly increased the risk of gingival recession. The type of orthodontic appliances also influenced outcomes, with fixed retainers associated with more pronounced recession compared to removable retainers. Some studies demonstrated effective management of gingival recession through specific orthodontic procedures, while others reported minimal effects, highlighting the need for individualized treatment planning and further research.

Conclusion : The relationship between orthodontic treatment and gingival recession is influenced by several factors, including the degree of tooth proclination and the type of orthodontic appliances used. While some studies indicate a clear association between orthodontic treatment and increased gingival recession, others report minimal or no significant effects. Careful treatment planning and individualized patient assessment are essential to minimize the risk of gingival recession. Further well-controlled, long-term studies are needed to elucidate the mechanisms and risk factors involved and to develop preventive strategies for optimizing gingival health during orthodontic treatment
